Information from your adoption record is available with positive identification of the individual requesting information (photo ID or notarized letter stating your identity). Due to the staff time required to locate the files and find the needed information, a fee of $50 will be charged for all services up to 1 hour of staff time and $50 for each additional hour thereafter. Names and any identifying information are strictly confidential and Utah law prohibits release of that information without consent.
