Utah law allows for non-identifying information from our files to be shared with you. This information will give you a history background on your birth family or the adoptive family of your birth child, including such things as physical descriptions, occupations, education, religion, nationality, siblings, hobbies, age, marital status, etc. It will also include medical information. The amount of information you may receive will depend wholly on how much information was given to the agency during the adoption process. The cost for this service is $75. The fee covers the cost of locating the appropriate files, reviewing them, and extracting all non-identifying information to share with you in a written format, as well as the time for discussion of any questions or issues you might have.
We can also provide a non-identifying report on medical information only for a fee of $50. Again, the amount of information you may receive will depend on how much information was given to the agency at the time of the adoption.
For either of these services, please complete the Adoptee Request for Non-Identifying Information form. Send it with your check or money order to:
Children's Service Society
Attn: Post-Adoption Worker
124 South 400 East, Suite 400
Salt Lake City, UT 84111
Your report will be mailed to you when it is completed.
